Job Summary: The Wraparound Facilitator follows the state's high-fidelity wraparound process with families, collaborating fully with team members to create a care plan, supervise its execution, and identify both formal and informal strategies to help families address their fundamental needs. The Facilitator ensures care is provided in a way that aligns with principles of strength-based, family-centered, and culturally competent practices. Additionally, the Facilitator educates all team members about the process's values, monitors progress in meeting families' underlying needs, and ensures that all documentation is completed in accordance with the specific wraparound program for which the youth is eligible.
7. Essential Job Duties: 
1. Work with families towards completing the four phases of the Wraparound
process through the development of wraparound teams and the establishment of a
comprehensive plan of care. 
2. Document all interactions with each family within the electronic medical record as well as under the wraparound program system in which the youth qualifies.
3. Develop and maintain positive relationships with youth, caregivers and teams in which the wraparound facilitator partners with.
5. Work with the Division of Mental Health and Addiction under Children’s Mental Health Wraparound as well the Department of Child Services Children’s Mental Health Initiative to ensure all pieces within the process are met with the high- fidelity model.
6. Complete all trainings as outlined by DMHA with the completion of additional workshops, boosters and trainings as required.
7. Follow the Family Health Center/ DMHA expectation of ratio of family to wraparound facilitator partnerships.
a. Required Education: Bachelor’s degree from an accredited college or university in social work, psychology, or related field.
b. Required Experience: Two years of experience with children and families.
